To help us get you started, we ask new clients to complete a short request form. This lets us confirm your insurance and be sure your needs are within our scope of care before scheduling your first appointment.

How It Works

1. Request Appointment (short form takes less than a minute).

2. We confirm your insurance & fit for medication management.

3. You complete full intake forms (sent securely through our Electronic Health Record).

4. First appointment: 60-minute initial evaluation via telehealth.

5. Follow-ups: Typically every 2 weeks at first, then monthly or every 3 months depending on your needs.
