THIS NOTICE DESCRIBES HOW HEALTH INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This notice applies to Rooted in Serenity Behavioral Health LLC and to Tannia Salazar, APRN, the independent practitioner responsible for your care.

We understand that information about you and your health is personal. We are committed to protecting your protected health information (PHI). We create and maintain records of the care and services you receive to provide quality care and comply with legal requirements.

Psychotherapy Notes: Psychotherapy notes are afforded special protection and will not be disclosed without your written authorization, except as permitted by law.
