At Rooted in Serenity Behavioral Health LLC, your privacy is very important to us. We are committed to safeguarding your personal and health information in accordance with federal and Connecticut law.

We only collect and use your health information to provide psychiatric care, process payments, and meet legal requirements. We will never sell your information. You have the right to:

  • Request access to your records.
  • Ask us to correct or update information.
  • Limit certain uses or disclosures.
  • Request a copy of this privacy notice at any time.

THIS NOTICE DESCRIBES HOW HEALTH INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This notice applies to Rooted in Serenity Behavioral Health and to Tannia Salazar, APRN, as the independent practitioner responsible for your care.

I. My Pledge Regarding Health Information

I understand that health information about you and your health care is personal. I am committed to protecting it. I create a record of the care and services you receive to provide quality care and comply with legal requirements. This notice applies to all records of your care.

  • Make sure that protected health information (“PHI”) that identifies you is kept private.
  • Give you this notice of my legal duties and privacy practices with respect to health information.
  • Follow the terms of the notice that is currently in effect.

II. How I May Use and Disclose Health Information

Treatment, Payment, and Operations: We may use or disclose PHI without written authorization to provide care, bill, or run our practice. For example, consulting with another provider about your condition.

Lawsuits and Disputes: We may disclose PHI in response to a court or administrative order, or certain legal processes with notice/protection.

III. Certain Uses and Disclosures Require Authorization

Psychotherapy Notes: Disclosure requires authorization unless for treatment, training, legal defense, HHS review, or by law.

Marketing / Sale of PHI: We will not use or disclose your PHI for marketing or sell your PHI.

IV. Certain Uses and Disclosures Do Not Require Authorization

  • When required by state or federal law
  • For public health activities (e.g., abuse reporting, safety threats)
  • For audits and investigations
  • For judicial/administrative proceedings
  • For law enforcement purposes
  • To coroners or medical examiners
  • For research purposes under law
  • For specialized government functions
  • For workers’ compensation
  • For appointment reminders and treatment alternatives

V. Uses and Disclosures That Give You the Opportunity to Object

We may provide PHI to family, friends, or others involved in your care or payment unless you object. Consent may be obtained retroactively in emergencies.

VI. Your Rights

  • Request Limits: Ask us not to use/disclose PHI for treatment, payment, or operations (not guaranteed if it affects care).
  • Restrict Disclosures: Limit disclosures to health plans if you pay out-of-pocket in full.
  • Choose Contact Method: Request specific communication preferences.
  • Access & Copies: Request electronic/paper copies of your medical record (fees may apply).
  • List of Disclosures: Request a list of disclosures made in the past six years (first list free each year).
  • Corrections: Request corrections or updates to PHI (if denied, you’ll receive an explanation).
  • Copy of This Notice: Request paper or electronic copy anytime.

Acknowledgement of Receipt

Under HIPAA, you have certain rights regarding the use and disclosure of your PHI. Engaging in care acknowledges receipt of this notice.
