These Terms of Service (“Terms”) govern your use of this website and any non-emergency services provided by Rooted in Serenity Behavioral Health LLC (“Rooted in Serenity,” “we,” “our,” or “us”). By accessing this website or receiving services, you agree to these Terms. If you do not agree, please do not use this website or services.
Who We Serve
- Services are provided to adults (18+) located in Connecticut at the time of service.
- We provide psychiatric medication management and related care within our clinical scope and licensure.

The information on this website is provided for general informational purposes only and is not medical advice. Website content does not create a patient-provider relationship. Do not delay or avoid care based on information on this website. If you believe you may be experiencing a medical or mental health emergency, call 911 or go to the nearest emergency room.
Telehealth Services
Our services are provided primarily through secure telehealth technology. By participating in telehealth, you consent to communicate electronically for evaluation, treatment planning, and psychiatric medication management.
Telehealth has benefits and limitations, including technology disruptions and limits to physical examination. If clinically appropriate, we may recommend an in-person visit or referral to another level of care.
Patient Responsibilities
- Provide accurate and complete health information, including medical history and current medications.
- Notify us of changes in symptoms, medications, allergies, or health status.
- Maintain the confidentiality of your patient portal and telehealth login credentials.
- Use services appropriately and respectfully. Disruptive, abusive, or unsafe behavior may result in termination of services.
- Attend scheduled appointments or provide reasonable notice to cancel or reschedule.
Non-Emergency Care Only
Rooted in Serenity provides non-emergency outpatient services. Our practice is not an emergency service and does not provide 24/7 crisis coverage. If you are in crisis or feel at risk of harm, call 911, 988, or 211 (CT resources) immediately.
Appointments, Cancellations, and Fees
Fees for missed appointments, late cancellations, or unpaid balances may apply. Applicable fees and policies are provided during onboarding and/or posted on our website or patient portal. By scheduling care, you agree to follow these policies.
Payment and Insurance
- If we accept your insurance plan, you are responsible for copays, deductibles, coinsurance, and balances not covered by insurance.
- If you are self-pay, payment is due at the time of service unless otherwise arranged in writing.
- Nonpayment may result in suspension or termination of services, as permitted by law.
Third-Party Platforms and Links
We may use third-party platforms (such as patient portals, telehealth technology, forms, or payment processors) to provide services. Your use of those platforms may be subject to their own terms and privacy practices. This website may also include links to external websites for convenience; we do not control and are not responsible for the content or practices of third-party sites.
