These Terms of Service (“Terms”) govern your use of the website and telehealth services provided by Rooted in Serenity Behavioral Health LLC (“we,” “our,” “us”). By accessing our website or receiving services, you agree to these Terms. Please review them carefully.

Use of Services

Services are provided to adults (18+) who reside in Connecticut.
You agree to provide accurate, complete, and current information during registration and treatment.
You are responsible for maintaining the confidentiality of your login credentials for any patient portals or telehealth platforms used.

Telehealth Services

Our services are provided primarily through secure telehealth technology. By using our telehealth platform, you consent to the use of electronic communications for psychiatric medication management.

While telehealth is effective, limitations exist (e.g., technology issues, inability to perform physical exams). We may recommend in-person care if clinically necessary.

Patient Responsibilities

Provide accurate medical history and keep us updated about medications, conditions, and changes in health status.
Attend scheduled appointments or cancel/reschedule with reasonable notice.
Understand that missed appointments or late cancellations may result in fees.
Use services appropriately and not for emergencies. In a crisis, call 911, 988, or 211 (CT Mobile Crisis Response).

Payment and Insurance

We accept select insurance plans as listed on our website. You are responsible for copays, deductibles, or any balances not covered by insurance.
Self-pay patients agree to pay fees at the time of service unless otherwise arranged.
Failure to pay may result in suspension of services until balance is resolved.

Limitations of Liability

Our services are provided “as is” and “as available.” While we strive for accuracy and reliability, we make no warranties that services will always be uninterrupted, error-free, or meet all expectations. To the fullest extent permitted by law, Rooted in Serenity Behavioral Health LLC disclaims liability for damages arising from use of our website or services.

Termination of Services

We reserve the right to terminate or suspend services if a patient fails to follow treatment recommendations, engages in inappropriate behavior, violates these Terms, or fails to meet financial responsibilities. Patients may also choose to terminate services at any time by providing notice.

Changes to These Terms

We may update these Terms periodically. The latest version will always be posted on our website with the effective date. Continued use of our services after changes indicates acceptance of the updated Terms.
