At Rooted in Serenity Behavioral Health LLC, we provide outpatient psychiatric medication management for adults across Connecticut, with an integrative medical approach that considers how lifestyle, physical health, and emotional well-being are interconnected.

Care is grounded in evidence-based psychiatric practice and delivered through a collaborative, thoughtful process. Appointments are intentionally paced to allow for careful assessment, clear treatment planning, and ongoing follow-up. Services are offered through secure telehealth statewide and in-person appointments in Middlebury, CT.
We currently accept self-pay and most major insurance plans, including UnitedHealthcare, Optum, Aetna, Anthem | BCBS, Carelon, Cigna, ConnectiCare, Oxford, and HUSKY Health (CT Medicaid).

Personalized Medication Management
Treatment is grounded in evidence-based medication management tailored to your symptoms, goals, and response to treatment. When appropriate, we also consider lifestyle factors such as sleep, stress, and physical health that may influence how medications work and how you feel on them.
Ongoing Follow-Up & Symptom Monitoring
Follow-up visits provide space to assess progress, adjust medications as needed, and address questions or concerns over time. Appointments focus on symptom tracking, medication response, and maintaining stability through clear, collaborative care.

Initial Psychiatric Evaluation
Your first appointment is a comprehensive psychiatric evaluation focused on understanding your current concerns, medical history, and treatment goals. We review relevant mental health history, medications, physical health factors, sleep patterns, and lifestyle considerations that may be influencing symptoms.
This initial visit is designed to clarify diagnosis, discuss treatment options, and determine an appropriate medication management plan. Care is evidence-based, collaborative, and tailored to your individual needs.
Duration & Fees
Initial psychiatric evaluations are typically scheduled for 60 minutes, though session length may vary based on clinical complexity.
Self-pay rate: $325
Insurance: If using in-network insurance, costs depend on your plan’s copay, coinsurance, or deductible
Out-of-network: Superbills are available upon request
Follow-Up Appointments
Follow-up appointments focus on monitoring progress, assessing medication response, and making adjustments as needed. We review changes in symptoms such as mood, focus, sleep, or energy, and address relevant medical or lifestyle factors that may affect treatment.
When appropriate, visits may include review of lab results or discussion of physical health considerations that intersect with psychiatric care. The goal of follow-up visits is to support stability, clarity, and ongoing treatment optimization.
Duration & Fees
Follow-up appointments are typically scheduled for 30 minutes, though visit length may vary depending on clinical needs.
Self-pay rate: $220 - $265
Insurance: If using in-network insurance, costs depend on your plan’s copay, coinsurance, or deductible
Longer or more complex visits may be billed under a higher CPT code and may result in an additional charge.
Out-of-network: Superbills are available for clients using out-of-network benefits

Self-pay visits are not billed to insurance. Diagnostic codes and treatment details are not automatically shared with third parties unless you choose to request documentation.

Good Faith Estimates (GFE)
In accordance with the No Surprises Act, all self-pay patients receive a Good Faith Estimate prior to starting care. This estimate outlines the expected cost of services so you can make informed decisions about your care.
Out-of-Network Options
At Rooted in Serenity Behavioral Health, you may choose to use your out-of-network (OON) insurance benefits for psychiatric medication management services.
If your plan includes out-of-network behavioral health coverage, you may be eligible for partial reimbursement. After each visit, we can provide a superbill—an itemized receipt that includes the required CPT and diagnosis codes—which you may submit directly to your insurance company.
Reimbursement amounts vary by plan and are determined by your insurer. We recommend contacting your insurance provider in advance to ask about:

Rooted in Serenity Behavioral Health is an outpatient practice providing psychiatric medication management for adults. As a solo provider practice, we do not offer emergency services, after-hours coverage, or crisis intervention.
If you are experiencing a psychiatric emergency—such as thoughts of harming yourself or others, severe or urgent medication reactions, or a rapid worsening of symptoms—please seek immediate care through one of the following resources:

Scope of Care & Higher Levels of Support
Rooted in Serenity Behavioral Health provides outpatient psychiatric medication management for adults. While care is attentive and collaborative, some clinical situations require a higher level of support than can be safely provided in an outpatient or telehealth setting.
If at any point symptoms indicate that additional structure or monitoring is needed, a referral to a higher level of care may be recommended, such as:
Intensive Outpatient Programs (IOP)
Partial Hospitalization Programs (PHP)
Inpatient psychiatric care
These recommendations are made thoughtfully and with patient safety as the primary consideration.
Please note that Rooted in Serenity Behavioral Health does not provide crisis services, inpatient coordination, or 24/7 on-call care. If ongoing needs fall outside the scope of this practice, we will communicate this clearly and support an appropriate transition to a setting better equipped to meet those needs.
