We currently accept self-pay and most major insurance plans, including UnitedHealthcare, Optum, Aetna, Cigna, Anthem BCBS, ConnectiCare, Oxford, and HUSKY Health (CT Medicaid).

  • Your first appointment is a welcoming, judgment-free space where you can be heard and understood. We’ll review your mental health history, current concerns, medical background, and personal goals to build a complete understanding of your wellbeing.

    This comprehensive, trauma-informed evaluation helps clarify your needs, define treatment goals, and discuss care options—including medication management and holistic strategies when appropriate. Every step is collaborative, evidence-informed, and tailored to you.

  • Initial psychiatric evaluations are typically scheduled for 60 minutes, though session length may vary based on clinical need.

    The self-pay rate is $325.
    If you’re using in-network insurance, your cost will depend on your plan’s copay, coinsurance, or deductible.

    Superbills are available upon request for clients using out-of-network benefits.

Next Steps
  • Follow-up appointments provide a grounded space for reflection and refinement. We’ll review how you’ve been feeling, discuss any changes in mood, focus, or energy, and explore factors like sleep, nutrition, movement, and stressthat can influence your wellbeing.

    Longer or more complex visits may be billed under a higher CPT code and may result in an additional charge.

    If you’re using in-network insurance, your cost will depend on your plan’s copay, coinsurance, or deductible.

    Appointments that require additional time or higher clinical complexity—even if completed within 40 minutes—may be billed under a higher CPT code and may result in an additional charge.

    Superbills are available for clients using out-of-network benefits.

Good Faith Estimates (GFE)

In accordance with the No Surprises Act, all self-pay clients receive a Good Faith Estimate before beginning care, outlining the expected total cost of services.

Out-of-Network Options

At Rooted in Serenity Behavioral Health, you can choose to receive care through your out-of-network (OON) insurance benefits. This option allows for greater privacy, flexibility, and collaboration in your treatment—keeping your care focused on what helps you most.

If your plan includes out-of-network mental health benefits, you may be eligible for partial reimbursement. After each session, we’ll provide a superbill—an itemized receipt with the necessary CPT and diagnosis codes—so you can submit it directly to your insurance for possible reimbursement.

We recommend checking with your insurance provider for details about your out-of-network deductible, reimbursement rate, and claim process.

Rooted in Serenity Behavioral Health is an outpatient practice that offers psychiatric medication management for adults with mild to moderate mental health concerns. As a solo provider, I do not offer emergency, after-hours, or crisis intervention services.

If you are experiencing a psychiatric emergency—including thoughts of harming yourself or others, severe medication side effects, or a rapid decline in your mental health—please seek immediate care by calling 911 or going to your nearest emergency room. You can also access 24/7 support through:

Scope of Care & Higher Levels of Support

Trauma-informed Care, Emotional Safety | Rooted in Serenity Behavioral Health | Middlebury, CT

While we strive to provide attentive and collaborative care, some needs may require more intensive treatment than cannot be safely provided in a telehealth outpatient setting. If at any point your symptoms indicate a need for greater support, I may recommend a higher level of care, such as:

  • Intensive Outpatient Programs (IOP)

  • Partial Hospitalization Programs (PHP)

  • Inpatient psychiatric care

These referrals are always made with your safety and well-being in mind.

Please note that Rooted in Serenity does not provide crisis services, inpatient coordination, or 24/7 on-call care. If your ongoing needs fall outside the scope of this practice, we will support a smooth transition to a setting that can best meet those needs.
