Hi, I’m Tannia Salazar, APRN, FNP-BC, founder of Rooted in Serenity Behavioral Health.

I’m a board-certified Family Nurse Practitioner specializing in psychiatric medication management for adults, with a clinical background shaped by emergency medicine, trauma surgery, psychiatric care, and education. My experience as an educator continues to influence how I practice—prioritizing clarity, structure, and helping patients understand their care.

Before specializing in psychiatric medication management, I worked extensively in acute care settings treating medical crises. That experience continues to shape how I approach care today—steady under pressure and detail-oriented. Over time, I saw how often mental health difficulties persisted long after the immediate issue had passed, which led me to transition into psychiatric care.

Today, I focus on working with adults navigating ADHD, anxiety, stress, and mild to moderate mood concerns. My approach is individualized but practical—looking at the full picture, including sleep, stress, physical health, and day-to-day demands, and translating that into clear, manageable treatment plans.

As a Family Nurse Practitioner, I integrate both mental and physical health into care. When appropriate, this may include reviewing labs, considering lifestyle factors, and coordinating with your therapist or primary care provider.

Sessions are structured but not rigid. We focus on medication evaluation, symptom management, and clear next steps—without overcomplicating things. I place a strong emphasis on psychoeducation, so you leave appointments understanding your symptoms, your medications, and the plan moving forward.

My approach is grounded in evidence-based practice and guided by clarity, consistency, and respect. Care here is direct, thoughtful, and easy to engage with—supportive without being overwhelming.

Care is culturally responsive and LGBTQIA+ affirming, with an emphasis on professionalism and individualized treatment.

Care is provided through a hybrid model, with secure telehealth available statewide across Connecticut and in-person appointments in Middlebury. This practice is best suited for adults seeking outpatient, non-urgent psychiatric medication management for mild to moderate concerns.
