Hi, I’m Tannia Salazar, APRN, founder of Rooted in Serenity Behavioral Health.
I’m a board-certified Family Nurse Practitioner specializing in psychiatric medication management for adults, with a clinical background shaped by emergency medicine, trauma surgery, and psychiatry. I created this practice to offer something I believe is essential — but too often missing in health care — a space where people can feel heard, understood, and supported without judgment.

Before specializing in psychiatry, I worked extensively in acute care settings treating medical crises. Over time, I saw how often difficulties with focus, anxiety, and mood persisted long after the body had stabilized. My transition into psychiatry was a natural extension of that work — applying the same clinical vigilance, steadiness, and attention to complexity to mental health care.

Today, my work focuses on supporting adults navigating ADHD, focus, anxiety, stress, and mild to moderate mood-related concerns. I take a careful, individualized approach to evaluation and treatment, recognizing that symptoms are often influenced by multiple factors, including sleep, stress, physical health, and daily demands.

As a Family Nurse Practitioner, I integrate both mind and body into care. When appropriate, this may include reviewing relevant labs, discussing lifestyle factors, or coordinating with your therapist or primary care provider to support your overall health — not just isolated symptoms.

Each session includes thoughtful assessment, psychoeducation, and collaborative treatment planning. I take time to listen carefully and work alongside patients to develop plans that reflect their goals, pace, and priorities.

My approach is grounded in evidence-based practice and shaped by presence, clarity, and consistency. I aim to provide care that feels steady, respectful, and responsive — offering support without pressure or judgment. Care is culturally responsive and LGBTQIA+ affirming, with an emphasis on collaboration, respect, and individualized treatment.

Care is provided through a hybrid model, with secure telehealth available statewide across Connecticut and in-person appointments in Middlebury. Whether you are seeking support for attention, anxiety, or medication management, I aim to offer a space where care feels grounded and sustainable.
