Hi, I’m Tannia Salazar, APRN, the founder of Rooted in Serenity Behavioral Health LLC.
I’m a board-certified family nurse practitioner with a professional background shaped by the fast-paced worlds of trauma surgery, emergency medicine, and psychiatric care. I created this practice to offer something I believe is essential—but too often missing—in health care: a space to feel heard, understood, and supported without judgment.

Before specializing in psychiatry, I spent years in acute and high-acuity care, treating the physical aftermath of crisis. What stayed with me was how often the emotional wounds remained unspoken—how trauma can linger long after the body stabilizes. My transition into psychiatry wasn’t a departure from that work, but a continuation of it.

Mental health care demands the same vigilance and skill as acute care medicine—but it also calls for softness, attunement, and time. Today, I bring that same steadiness and presence into every session—applied now to healing what can’t always be seen.

As a Family Nurse Practitioner, I integrate both mind and body into care—recognizing how sleep, stress, hormones, and physical health all influence mental well-being. When appropriate, sessions may include lab reviews, wellness discussions, or coordination with your therapist or primary care provider to ensure that treatment supports your whole health, not just your symptoms.

At Rooted in Serenity, we provide trauma-informed, LGBTQIA+ affirming psychiatric medication management for adults navigating anxiety, depression, ADHD, and the long arc of healing from trauma.
Our practice is culturally sensitive, queer-friendly, and collaborative—grounded in both evidence and empathy. I draw on a strong clinical foundation to deliver thoughtful, personalized care that balances science with humanity, and structure with flexibility.

Each session includes a comprehensive evaluation, psychoeducation, and open, honest dialogue about your goals, symptoms, and strengths. I take time to truly listen and co-create treatment plans that reflect your story, your pace, and your voice.

My work is guided by the belief that mental health treatment should feel safe, inclusive, empowering, and responsive.

I offer care through a hybrid model—secure, HIPAA-compliant telehealth statewide and in-person appointments in Middlebury, CT—with a focus on accessibility, continuity, and compassion. Whether you’re starting fresh, transitioning care, or simply ready to feel more grounded, I’m here to offer a safe and affirming space to begin again.
