Hi, I’m Tannia Salazar, APRN, founder of Rooted in Serenity Behavioral Health.
I’m a board-certified Family Nurse Practitioner with a professional background shaped by the fast-paced worlds of trauma surgery, emergency medicine, and psychiatric care. I created this practice to offer something I believe is essential — but too often missing in health care — a space where people can feel heard, understood, and supported without judgment.

Before specializing in psychiatry, I worked extensively in acute care settings, treating the physical consequences of medical crisis. Over time, it became clear how often emotional and cognitive symptoms persisted long after the body stabilized. My transition into psychiatry was not a departure from that work, but a natural extension of it — applying the same clinical vigilance, steadiness, and attention to complexity to mental health care.

Mental health care demands the same level of care and precision as acute medicine — but it also requires presence, attunement, and time. Today, I bring that same steadiness into each session, focusing on careful assessment, thoughtful treatment planning, and support for what cannot always be seen on a monitor or lab report.

As a Family Nurse Practitioner, I integrate both mind and body into care, recognizing how sleep, stress, hormones, medications, and physical health all influence mental well-being. When appropriate, sessions may include review of relevant labs, wellness discussions, or coordination with your therapist or primary care provider to ensure treatment supports your overall health, not just isolated symptoms.

At Rooted in Serenity Behavioral Health, I provide outpatient psychiatric medication management for adults navigating ADHD, anxiety, depression, and PMDD. Care is culturally responsive, LGBTQIA+ affirming, and collaborative — grounded in evidence-based practice and delivered with empathy, intention, and respect.

Each session includes a comprehensive evaluation, psychoeducation, and open, honest discussion about symptoms, goals, and treatment options. I take time to listen carefully and work alongside patients to develop plans that reflect their needs, pace, and priorities.

My work is guided by the belief that psychiatric care should feel steady, inclusive, and responsive — offering clarity and support without judgment or pressure.

Care is offered through a hybrid model, with secure, HIPAA-compliant telehealth available statewide across Connecticut and in-person appointments in Middlebury, CT. Whether you are beginning care, transitioning providers, or seeking a more thoughtful approach to medication management, I aim to offer a space where care feels grounded and sustainable.
