Rooted in Serenity Behavioral Health LLC provides psychiatric evaluations and medication management services for adults located in Connecticut at the time of treatment. All services are delivered via secure, HIPAA-compliant telehealth.
Treatment may include psychiatric diagnostic impressions, psychoeducation, risk-benefit discussions, and prescription management when clinically appropriate.

I work exclusively with adults (18+) who are physically located in the state of Connecticut during each telehealth session. I welcome clients from all backgrounds and provide culturally sensitive, trauma-informed psychiatric care.

This practice focuses on the psychiatric evaluation and medication management of the following mental health concerns:

Attention-Deficit/Hyperactivity Disorder (ADHD)
Acute Stress Disorder

Treatment recommendations are personalized and based on a comprehensive assessment of your history, symptoms, and goals. Some conditions may require referral to a higher level of care if medication management alone is not appropriate.

This practice does not provide treatment for:
Bipolar I or 2 disorder
Schizophrenia or other psychotic disorders
Eating disorders
Borderline or antisocial personality disorder
Substance use disorders
Autism spectrum disorder
Cognitive impairment, dementia, or neurocognitive decline
Severe obsessive-compulsive disorder or tics
Primary neurological conditions or complex medical illness requiring coordination
If it becomes clear during evaluation that your care needs fall outside the scope of this practice, I will notify you and recommend you follow up with your primary care provider or insurance carrier for appropriate referrals.

No. I do not provide psychotherapy, counseling, or behavioral health case management. I focus solely on psychiatric assessment and medication management. Individuals seeking talk therapy or non-pharmacologic treatment options should consult a licensed therapist or use their insurance carrier's provider directory.

Yes. You may complete the Pre-Screening Form to determine eligibility. All new clients must meet the inclusion criteria for telehealth psychiatric services in Connecticut.

The initial intake visit is a 60-minute comprehensive psychiatric evaluation. We will review:
Presenting symptoms and treatment goals
Relevant medical, psychiatric, and family history
Past medication and/or therapy experiences
Risk factors and safety considerations
Your current needs, strengths, and concerns
By the end of the appointment, I may offer a diagnostic impression and initial treatment plan, which could include medication recommendations, lab work, or follow-up scheduling.
Please note: Completion of an intake appointment does not guarantee continuation of care. Ongoing treatment is contingent on clinical appropriateness and alignment with the practice scope.

Yes. Connecticut law requires that both provider and patient be physically located in Connecticut at the time of each telehealth visit. Appointments may be rescheduled or canceled if this requirement is not met.

Stimulants may be prescribed only when clinically appropriate, after a comprehensive psychiatric evaluation and review of prior records. Prescribing decisions are based on clinical judgment, state and federal prescribing regulations, and your individual risk profile.
If you are currently prescribed a stimulant by another provider, transfer of care is not guaranteed and will require full review and medical necessity

This practice does not prescribe benzodiazepines for long-term, daily, or routine use (e.g., Xanax, Ativan, Klonopin, or Valium). If benzodiazepines are part of your existing treatment, we may discuss safer alternatives in alignment with current clinical guidelines and public health recommendations.

You are never obligated to begin medication. If medication is not your preferred path, we can explore non-pharmacologic options when clinically appropriate. However, this practice does not offer therapy or lifestyle-based interventions alone.

Yes. I offer ADHD screening, diagnostic impressions, and medication management for adults. Formal neuropsychological testing is not provided in this setting but may be recommended based on complexity. If needed, please contact your PCP or insurance for referrals to testing services. Our resources page also lists some options, but please keep in mind this is not a comprehensive list.

Yes. I work with individuals experiencing symptoms related to past trauma. While I do not provide therapy, medication can be used to manage symptoms such as hyperarousal, panic, nightmares, and sleep disruption.
Medication is not a substitute for therapy, and you may benefit from concurrent counseling with a licensed psychotherapist.

Follow-ups are used to assess treatment response, side effects, and symptom progress. We’ll review:
Mood, anxiety, sleep, or attention changes
Medication tolerability and effectiveness
Psychosocial or medical changes affecting your care
Adjustments are made based on clinical need, with an emphasis on safety and sustainable outcomes.

If your insurance is out-of-network, you may self-pay and request a superbill for potential reimbursement. Please contact your insurance provider directly to confirm eligibility and benefits.

Self-pay rates are listed on the service page. I provide a Good Faith Estimate (GFE) upon request and as required by the No Surprises Act for clients who are uninsured or opting to pay out of pocket. Sliding scale is available with proof of income.

Appointments must be canceled or rescheduled with at least 24 hours’ notice. Late cancellations and missed appointments are subject to an $85 fee, which is not covered by insurance. Repeated missed or rescheduled appointments and policy violations will result in discharge from the practice.

Yes. As an independent provider, I reserve the right to discontinue care if:
Your needs exceed the scope of this practice
Policies are repeatedly violated
There is evidence of clinical risk that cannot be safely managed
You will be notified in writing if care is being discontinued. At that time, you may request documentation to share with your next provider.

I do not complete disability applications, emotional support animal letters, or forms requiring functional assessments outside the scope of psychiatric medication management.
For continuity of care and a comprehensive understanding of your overall health, these forms are often better suited to your primary care provider (PCP) or a treating therapist, who can assess your overall functioning and day-to-day needs in greater depth.

Rooted in Serenity Behavioral Health does not provide forensic evaluations, custody assessments, or court-mandated treatment. I do not participate in legal proceedings or provide expert testimony.
If you are involved in a legal matter that requires documentation, court appearance, or deposition, please be aware that:
I do not accept patients for the purpose of satisfying court-ordered treatment.
If I am subpoenaed or compelled to appear in court or submit records, you will be responsible for all associated fees, including:
Preparation time
Travel
Legal consultation
Court appearance time
These costs are not covered by insurance and will be billed at a rate of $250/hour, including travel and wait time.
If you are currently involved in a legal proceeding, I recommend seeking care from a provider who specializes in forensic or court-related services.

Rooted in Serenity Behavioral Health, LLC is a solo psychiatric medication management practice led by a board-certified Family Nurse Practitioner. All services are provided via telehealth to adult clients (ages 18 and over) who are physically located in Connecticut at the time of each session.

No therapy services are provided. This practice does not offer psychotherapy, counseling, or behavioral health case management. If you are seeking talk therapy or non-medication support, please consult a licensed therapist or explore options on the resources page.
No emergency or crisis services are available. This practice is not designed to provide urgent or emergent psychiatric care. If you are experiencing a mental health crisis, please contact 911, 988 (Suicide & Crisis Lifeline), or 211 (Connecticut Mobile Crisis Services).
No direct referrals are offered. While treatment needs may be discussed during your appointment, this practice does not provide direct referrals to therapists, specialists, or testing providers. You are encouraged to consult your primary care provider or insurance plan for appropriate referrals.
Care coordination is limited. You may request that documentation or treatment summaries be shared with your therapist, PCP, or other clinicians by submitting a signed release of information. Brief communication is generally included as part of care; however, extended coordination (e.g., phone calls, meetings, frequent provider communication) will incur a fee depending on time and complexity. Ongoing collaboration is offered at the provider’s discretion and may not be clinically indicated in all cases. This practice does not provide case management services.
Prescribing scope is limited. This practice does not initiate or continue prescriptions for benzodiazepines or certain other controlled substances when their use falls outside of established safety guidelines. Long-term use of medications such as Xanax, Klonopin, Ativan, and certain sleep aids may not be supported. All prescribing decisions are made individually based on clinical judgment, patient history, and applicable federal and state guidelines.

Engaging in an intake appointment does not guarantee a diagnosis, prescription, or continued treatment. Ongoing care is determined by clinical appropriateness, provider discretion, and mutual agreement. Clients may be discharged if treatment needs exceed the scope of care provided, or in cases of safety concerns, policy violations, or non-adherence.
