At Rooted in Serenity Behavioral Health, psychiatric care is grounded in compassion and informed by an integrative, trauma-informed approach. Whether through medication management, lifestyle guidance, or collaboration with your primary care team, care is personalized to help you feel balanced, understood, and supported.
Tannia Salazar, APRN, FNP-BC, founder of Rooted in Serenity Behavioral Health, offers holistic psychiatric medication management for adults navigating anxiety, depression, ADHD, and the long arc of healing from trauma. Her approach is collaborative, patient-centered, and grounded in respect for each individual’s story.
We take time to truly listen, assess thoughtfully, and partner with patients to co-create treatment plans that are evidence-based yet deeply personalized. Psychiatric care should feel safe, empowering, and responsive—never rushed or transactional. Sessions include medication evaluation and management, psychoeducation, and open dialogue around wellness, resilience, and goals.
With a background in family medicine, Tannia brings a whole-person perspective to psychiatric care and is especially passionate about supporting individuals whose experiences may have been overlooked or misunderstood in traditional settings. Through a hybrid model of secure telehealth across Connecticut and in-person psychiatry in Middlebury, CT, Rooted in Serenity Behavioral Health provides care focused on accessibility, continuity, and compassion.
Healing begins where safety is felt and trust is built. That’s where we start—together.

Our Philosophy
At Rooted in Serenity Behavioral Health, we believe effective psychiatric care begins with truly seeing the person in front of us—their story, strengths, and experiences. Our approach blends the science of psychopharmacology with the art of listening, creating space where emotional and physical health are equally valued.
With a background in family medicine, Tannia Salazar, APRN approaches psychiatric care through a wider lens—considering how factors like sleep, stress, nutrition, hormones, and overall wellness contribute to mental health. This integrative perspective ensures that care feels comprehensive, grounded, and sustainable.
Because no two people are alike, treatment is never one-size-fits-all. Our philosophy is collaborative, holistic, and validating:
Collaborative — decisions are made together, not for you.
Holistic — care considers labs, physical health, life context, culture, and mental well-being.
Validating — you deserve to feel heard, respected, and understood in every interaction.
Above all, psychiatry should be compassionate and accessible. Whether through medication management, reviewing medical contributors to symptoms, or working alongside therapists and primary care providers, our goal is to create a safe, supportive space where you feel grounded and can move toward growth.
Our Services
Care at Rooted in Serenity Behavioral Health is integrative and collaborative. In addition to medication management, visits may include wellness discussions, lab reviews, and coordination with your therapist or primary care provider. This ensures that biological, emotional, and lifestyle factors are considered together—helping you find steadiness, clarity, and long-term balance. As a Family Nurse Practitioner, I bring a whole-person perspective to psychiatric care, recognizing how physical health, lifestyle, and emotional well-being are deeply interconnected.
We provide trauma-informed psychiatric medication management for adults navigating anxiety, depression, ADHD, and the ongoing journey of healing from trauma. Care is delivered through a hybrid model—secure telehealth across Connecticut and in-person appointments in Middlebury, CT—with an emphasis on accessibility, respect, and continuity of care.
Each session blends medication evaluation, psychoeducation, and supportive dialogue, creating treatment plans that are both evidence-based and deeply personal.
